At Evergreen Terrace, caring, friendly and professional staff come together to create a homelike environment that encourages dignity and independence and welcomes the active involvement of family members and friends.

We offer both long-term and short-term care. Short-term care is for those who require rehabilitation following hospitalization. Long-term care provides round-the-clock assistance, medical attention and security for those requiring more care. Our health center also provides a specialized environment for those who need memory care. Evergreen Terrace, on the south side of Grand Rapids, close to shopping, restaurants, and health care is attached to Pleasant Seasons Assisted living with 50 care suites, providing a total continuum care community.

Evergreen Terrace offers exceptional professional care, provided by physicians, nurses, rehabilitation therapists, social workers, a chaplain, activities therapists and other staff. Evergreen Terrace embraces a philosophy of interacting with residents called person centered care. We honor the choices, preferences and abilities of all residents while we pay special attention to our resident's individual needs and preferences. Our 114 bed, Medicare-approved health care center, attracts residents from Grand Rapids and surrounding communities.
