Evergreen Terrace's Memory Care Unit

Our skilled nursing center includes a newly remodeled separate unit dedicated to the care of residents with Alzheimer's disease and other forms of dementia. Our Memory Care Program is dedicated to "celebrating the spirit of our residents and maintaining the highest quality of life possible for each resident entrusted to our care."

This unit offers:
 • A secure unit for residents who wander

 • A comfortable and compassionate environment that emphasizes positive life
experiences and active involvement in a daily routine

 • Specially trained staff

 • A place where families are encouraged to remain integrally involved in the lives of
their loved ones

 • An ongoing program of meaningful, interesting and stimulation recreational
activities designed to promote physical, mental, spiritual and social well-being

 • A facility design maximizing the ability of residents to interact socially with each
other and their families in a safe environment

 • A unique and private outdoor walking garden

 • A place where staff seek to adapt to each individual's pattern of behavior so we
can provide the best possible environment for every one of our residents to be
successful in the tasks of day-to-day living

Evergreen Terrace's Memory Care Mission Statement:
"Dementia Friendly Care"

Evergreen Terrace's Memory Care Value Statement:
"We believe in the intrinsic value of each individual, despite the disease process.
We strive to enhance, enrich and empower every man and woman in our care to preserve his or her dignity, self-respect and optimum functioning. We will do this one-day at a time for as long as we are privileged to be part of their lives. This will be accomplished through an environment that is calm, safe and therapeutically structured. Our staff is dementia friendly, respectful and supportive of our residents and their families."

Memory Care Goals and Objectives:
 • Person Centered Care Resulting in Positive Self Esteem

 • Safe, Structured Environment

 • Anxiety Reduction

 • Success-Oriented Activities

 • Dementia Friendly Staff

 • Family Friendly Staff

 • Minimal Use of Restraints - Physical and/or Chemical

"Dignity is imperative for the survival of self. The Alzheimer's patient asks nothing more than a hand to hold, a heart to care, and a mind to think for them when they cannot; someone to protect them as they travel through the dangerous twists and turns of the labyrinth."
